I was reading McKinsey’s latest report on the state of AI and one point immediately stood out. Explainability is consistently cited by enterprises as a meaningful AI risk, yet it tends to receive lower priority compared with other concerns when organizations decide where to focus their mitigation efforts. This contrast seems small at first, but it raises a meaningful question. Why do some risks remain recognized, yet addressed more slowly in practice?

When a risk is well known but remains unresolved, the barrier is usually not awareness. It is that organizations do not have a practical way to respond. Explainability becomes something leaders recognize, but struggle to translate into operational action. This gap between recognition and action creates hesitation, and hesitation slows the path from pilot work to scaled use.


The issue is not the strength of the models themselves. Modern systems perform well in controlled settings. The challenge appears when enterprises try to place these systems inside environments that expect clarity, traceability, and accountable choices. Without structures that support these expectations, even high performing systems remain hard to operationalize at scale.


This raises the central question for this article. If the risks are known and the technology is strong, why does progress slow once organizations move beyond pilots? The answer points toward a structural challenge inside the enterprise rather than a technical one. These two issues may seem different, but they come from the same underlying challenge. AI produces output, but the path that leads to the output is not visible to the enterprise.


When the answer is wrong, the impact is direct and clear. It creates a loss of trust and raises questions about reliability. When the answer is right but cannot be explained, the impact is quieter but still significant. Leaders hesitate, teams slow down, and the system remains at the edge of key decisions. Both forms of uncertainty limit how far AI can enter the core of the business.


This is why explainability has become an important threshold for enterprise scale. It is not only a question of understanding the model. It is a question of whether the organization has the structures to trace, review, and take responsibility for decisions that involve AI. As long as these structures remain incomplete, adoption will continue to stall at the pilot stage. The barrier is not performance. The barrier is governance.


For many enterprises, technical capability is no longer the main constraint. Models continue to advance, and performance improves each year. What slows progress is the lack of a governance system that can support AI at scale. This is why leading organizations often begin with workflow redesign rather than model improvement. Workflows are where traceability is created, reviewed, and sustained. They form the foundation that allows intelligence to operate safely and reliably inside the enterprise.



How Governance Requirements Shape AI’s Path to Scale


Enterprises that work in high-risk domains face a basic set of questions every time a decision is made. They need to understand why a decision was taken, what information supported it, who should be accountable if the outcome changes, and whether the full process can be audited. These expectations are not special requirements. They are the foundation of responsible operations in fields such as healthcare, finance, insurance, and risk control.


When AI enters these environments, the expectations do not disappear. They become more important. Yet these are the areas where current LLM usage patterns show clear gaps.


Modern systems produce output, but they do not provide a reasoning trail that can be reviewed. The information sources behind an answer cannot be reconstructed in a reliable way. The steps that lead to the answer cannot be verified in a gradual or structured form. The output cannot be placed cleanly inside existing responsibility frameworks. These gaps do not reflect model performance. They arise because enterprise environments require forms of clarity that current AI systems are not designed to produce directly.


This is why AI remains difficult to place inside the core of high-stakes workflows. Medical diagnosis, risk assessment, lending decisions, supply chain planning, and insurance adjudication all require decisions that can be traced, challenged, defended, and reviewed. Without structures that support these expectations, even advanced systems remain limited to advisory roles. They can suggest, but they cannot act. They can inform, but they cannot replace human judgement where accountability is required.


The obstacle is not a lack of willingness to scale. Most enterprises want to use AI more deeply. The obstacle is that the governance structures needed to support large scale adoption have not yet been built inside the organization. These structures are not technical features of a model. They involve roles, processes, controls, and decision pathways that make it possible to place intelligence inside real operations.


This creates a clear implication. Enterprises do not struggle because the technology is unprepared. They struggle because the environment around the technology is not yet ready to carry it. And this gap becomes one of the strongest reasons why scaling AI remains difficult across industries.

Organizations can deploy a strong system, but without a way to trace decisions, assign responsibility, or integrate outcomes into existing controls, the model cannot take on meaningful operational roles. The gap is not technical. The gap sits in the architecture around the model.


As models continue to advance, the gap becomes more visible. Each improvement in capability highlights the distance between what the technology can do and what the organization is prepared to manage. Leaders may see the potential value, but they also see that the surrounding systems are not yet designed to support accountable use. As models grow more capable, they highlight the need for governance structures that can support new forms of intelligence. Performance moves forward quickly, while oversight frameworks evolve more gradually, and the space between the two becomes clearer over time.


For many enterprises, this is the point where scale breaks down. Pilot projects succeed because they operate within controlled boundaries. Once the system needs to touch real workflows, real customers, or real financial exposure, the questions around traceability and accountable decisions return. Without clear answers, progress slows. The issue is not reluctance. It is that the organization does not yet have the roles, processes, or safeguards that allow AI to operate at scale.


This is the essence of the governance gap. It is the space between what models can do and what enterprises can responsibly support. It explains why many efforts stall even when the technology works well. It also sets the stage for the shift that follows. Scaling AI is no longer about stronger models. Three Emerging Directions That Pull AI Toward Workflow Redesign


As organizations work to bring AI into real operations, a set of shared directions has begun to emerge across industry discussions, research dialogues, and enterprise practice. These directions do not focus on building more complex models. They focus on reshaping the structures that allow intelligence to function inside the enterprise. The shift is subtle but significant. It shows that scaling AI is not only a technical task, but an organizational one.


One of the clearest areas of convergence is workflow redesign.


Enterprises are finding that intelligence performs best when it sits inside workflows that are traceable, auditable, and ready for human handover when needed. The objective is not to make the model transparent. It is to make the workflow transparent. When teams can see where AI contributes, how decisions move through the process, and when humans take responsibility, uncertainty decreases. The bottleneck is less about model capability and more about the workflow’s ability to support accountable use. It is the workflow that determines whether intelligence can be applied in a consistent and reliable way.


A second direction is the growing emphasis on confidence signaling.


Enterprises do not need AI to present a full reasoning chain. They need a signal that shows how confident the system is in its output. This allows the workflow to respond differently depending on the level of certainty. High confidence may trigger automation, while low confidence may route the task to human review. This pattern is becoming more common across both practice and research. Confidence is not a model feature. It is a workflow mechanism that guides how work should flow when uncertainty is present.


The third direction centers on governance architecture.


The aim of governance is not to make the model more understandable. It is to make the environment around the model governable. This includes the rules, roles, controls, and pathways that determine how AI enters a workflow and how decisions are reviewed. Effective governance creates the structure that allows AI to operate without replacing accountability. Governance is not about managing AI itself. It is about managing how AI participates in the work of the organization.


Together, these three directions form a coherent structural shift. They point to a future in which workflows, not models, determine whether AI can scale.


Enterprises struggle to operationalize AI not because the models lack capability, but because the structures required to manage and scale intelligence have never existed. To build these structures, organizations must rethink how workflows are designed, how uncertainty is handled, and how responsibility is carried. Workflows must be redesigned to carry intelligence. This is the foundation on which scalable AI will be built.

What slows progress is rarely technical limitation. It is the lack of structures that help teams see how decisions are made, how uncertainty should be handled, and where accountability sits. Without these foundations, AI remains something that supports the workflow but never becomes part of it.


When workflows are redesigned with clarity and traceability in mind, AI begins to fit more naturally into daily operations. Teams know when to trust the system, when to intervene, and how to review outcomes. This shift turns AI from an experiment into a function the organization can depend on.


Enterprises struggle to operationalize AI not because the technology is lacking, but because the governance structures required to manage and scale intelligence have yet to be built.


Establishing these structures is now a strategic task. Once they are in place, AI can move from isolated success to meaningful influence across the enterprise.
